1)  Full names exactly as shown on valid passport for each guest per room
2)  Passport information (number, expiry date, etc)
3)  Birthdates for each guest
4)  Full mailing address including postal code
5)  Telephone numbers including area code
6)  Email Address
7)  Form of payment
